Policy

A specific process will be implemented to assure consistent appointment practices.


Appointment Policy

Appointments can be made directly by calling our office. Alternatively, patients can request an appointment through our patient portal or through our contact page. Patients are to bring their current insurance cards and medication list to all appointments. We will do our best to schedule appointments appropriately and timely. Maintaining appointment times is important, please be aware of the cancellation, late, and no-show portion of our appointment policy.


Cancellation Policy

We understand that things come up and that appointments may need to be canceled or rescheduled. We require 24-hours notice in the event you need to cancel or re-schedule. Failing to do so could inconvenience our medical team and our other patients delaying our ability to provide quality care. Failure to cancel or re-schedule an appointment within 24-hours may result in a charge to your account of $25. Insurance will not cover this charge. You will be required to pay this charge prior to scheduling any further appointments.


No Show Policy

Our schedule is busy and we try to accommodate all patient needs as they arise. Because of this, we ask that you notify us if you aren’t able to make your appointment 24-hours in advance so we can offer it to someone else who needs it. Missing an appointment without any notification inconveniences both our medical staff and our other patients. Missing two appointments within a 6-month period with no notification will result in a $50 charge to your account. Insurance will not cover this charge and you will be required to pay the $50 before any further appointments are scheduled. Excessive no-shows, 3 or more in a 6-month period, or 4 or more in a 12-month period, will result in being discharged from our office. New patient appointments that are missed will not be rescheduled unless the reason for not canceling or showing was unavoidable.


Late Policy

If you’re going to be late for your appointment we ask that you notify us as soon as possible. We will do our best to accommodate the late arrival, but depending on the schedule we may ask that you re-schedule. Arriving late to your appointment causes a delay in all patients scheduled after you and can inconvenience our medical staff. We work diligently to remain on schedule and ask that you arrive 15 minutes prior to your appointment for the necessary paperwork and check-in.
